Convert root `<hypothesis-app>` component to Preact

Convert the root `<hypothesis-app>` component to Preact and update the
startup code in `src/sidebar/index.js` to replace the AngularJS app
bootstrap with rendering the `HypothesisApp` component directly using
`render` from Preact.
