There is currently an issue in h where the search API call may sometimes return
pages that are incomplete, even if there are more pages of results to fetch
afterwards [1]. A non-full page caused the client to stop fetching more pages as
it incorrectly assumed it had reached the end. Ultimately this needs to be
resolved in the API [2] by adding an explicit next-page link. Until that is done,
change the client to continue paging through results as long as the current page
has at least one entry (needed to construct the cursor) and we expect more
results based on the `total` value in the response.

This causes TS to check assignability rather than casting the type, which can
catch additional mistakes, such as mis-naming an optional property.

Convert SearchClient to TS and improve the class documentation to better
explain its responsibilities.

After some additional testing it seems the original comment was incorrect and
that `index` is only set in PDF-based ("pbk") books.

These fields may not be missing from the response of VitalSource's
`getCurrentPage` API if the book is an EPUB that does not have page numbers. As
far as I know all PDF-based books should have page numbers.

If for any reason VitalSource's `<mosaic-book>` API does not return the page
info fields that the Hypothesis client is expecting, fail loudly so we find out
about the issue, rather than silently creating annotations with
missing/incorrect chapter location information.

I don't currently know of any situation where this is expected to happen, so
this is protection against future changes or differences in behaviors with
certain books that we have not observed yet.

In context, `AnnotationShareControl` is rendered within its associated
`ThreadCard`. `ThreadCard`s are focusable, so any click within the
panel that is not on a focusable element will "leak out" and focus the
annotation card. This will cause the panel to close because the focus
event is outside of the panel.

Prevent focus from leaking by making the panel itself focusable.

 - Remove the book_as_single_document flag, as we have now enabled this
   flag for everyone in production

 - Throw if the `isbn` property is missing from the book info returned
   by VitalSource. If this ever happens, we want to fail loudly instead
   of silently capturing a URL with no book ID on it.
