This should give us a way to trigger CI for a PR if GitHub has failed to
do so for any reason.

Our updated shared `Input` component will bleat (console warning) if
it's not given either an `id` or an `aria-label`. Add `aria-label` here.

Clone instead of move SVG highlights when focusing such that their
original order is not lost when they are subsequently unfocused. Ensure
that cloned, focused highlights are removed when associated highlights
are removed.

`Object.fromEntries` is showing up in Sentry as the most frequent error from
browsers [1] that are outside our supported range, so use it as a representative
feature to decide whether the client can load at all. We assume that this can
subsume the previous ES2015-era feature tests.

`Object.fromEntries` is available since Chrome 73 (Mar 2019), Safari
12.1 (Mar 2019), Firefox 63 (Oct 2018).

The `getSegmentSelector` function assumed all annotations have at least one
target. This is true for all saved annotations and all new annotations with
selectors, but is not true for new page notes.

This is a stop-gap fix. The better solution will be to eliminate the
inconsistency and make it so that all annotations have a non-empty `target`
field.

When the client is unloaded in a guest-only iframe, remove the client
configuration that was added by the `injectClient` function. This is
necessary for subsequent reloading of the client to work, as
`injectClient` aborts if the configuration already exists in a frame.

This change allows the client to be properly de-activated and then later
re-activated in a VitalSource book.

When the client is unloaded in a frame H, ensure that it is also unloaded from
guest frames whose host frame is H. This enables toggling the browser
extension's active state to work properly in VitalSource Bookshelf and other
pages which have annotation-enabled iframes.

This works by having the `Guest` instance in the iframe listen for the existing
"close" message from its connection to the host frame, and relaying that to the
entry point via a "hostDisconnected" event, which then handles it in the same
way as if the client had been unloaded directly in the guest frame.
