These files were not formatted previously due to the Prettier glob not matching
`*.mjs`.

Now that the package type is set to `module`, we don't need to use a custom
extension to indicate that a script is ESM.

This was missed in 7308187e. This used to rely
on `require` determining the boot script URL from the `main` field in
package.json, but the indirection is a bit confusing, so just reference the
output URL of the boot script directly.

This improves the overall consistency of the JS environment and simplifies
config for linting etc. Karma is the exception as module scripts are not
supported for its config.
