Remove the highlighter implementation abstraction layer in
src/annotator/highlighter since only one type of highlighter was ever
implemented and convert the one implementation in
src/annotator/highlighter/dom-wrap-highlighter/index.coffee to JS as
src/annotator/highlighter.js.
